The Pathways Hub, newly developed through Better Health Partnership housed out of Metro Health Medical Center, is a model that brings together clinical, community and other service agencies to mitigate issues such as chronic disease, infant mortality, mental health etc... There process and end outcomes collected and reimbursement through managed care organizations of positive outcomes achieved. The Pathways Hub model uses community health workers as navigators as an integral part of the model.
