Center for Disease Control: Racial and Ethnic Approaches to Community Health (CDC:REACH).

In Year Five the CSU School of Nursing, which houses the Community Health Worker (CHW) Training Program, will not run a new cohort. Instead there will be added focus on assisting the first four cohorts to complete service hours (curtailed due to the pandemic), an increased focus on program evaluation metrics and an increased focus on placement opportunities with the other arms of the grant which are physical activity and nutrition.
