Description

Centers for Disease Control (CDC) through Cuyahoga County Board of Health Improvement Partnership (HIP-Cuyahoga) Racial and Ethnic Approaches to Community Health (REACH). The Cleveland State University School of Nursing’s Ohio Board of Nursing (OBN) certified Community Health Worker (CHW) Program which has dual training as group leaders for the Stanford Chronic Disease Self-Management Progra fall under the Community Linkage area of the overall REACH grant. Other partners under this area are Metro Health, Fairhill Partners, United Way and A Vision of Change. This $900,000.00 proposal was submitted and was awarded. The CSU SON CHW program is one of eleven programs funded with this five year initiative.
