Cleveland leaders speak about programs that showcase efforts of collaborations and partnerships between sectors of the community Objectives: List different community entities that can collaborate toward positive health outcomes Explain how partnerships can improve the community's health. Dr. Joan Thoman's part was to speak to the Cleveland State University School of Nursing's Community Health Worker Program in which participants obtain dual certification as a Community Health Worker and a Stanford Chronic Disease Self-Management Group Leaders
